On Lok PACE East San Jose Center is a full-service healthcare program that provides senior care that supports seniors to remain living at home. Our PACE program includes medical care and senior services at your home and at our PACE centers, including transportation services for seniors, memory care, meal delivery for seniors, social activities, home care, vision, dental, and hearing. Our team of healthcare professionals creates individualized care plans to meet your needs, which are covered by Medi-Cal and Medicare. When enrolled in On Lok PACE, your services must be received through On Lok contracted providers or you could be personally liable for costs incurred, unless it is an emergency or urgent situation.
